A 45 years old female patient as admitted to . Seoul Red Cross Hospital through the emergency room with chief compliant of severe headache, dizziness, and muscle weakness of upper an lower extremities. Her past history was revealed hypertension for 8 years but has not been treated. Physical examination showed that she had hypertension, positive Trousseau´s sign, and diminished deep, tendon reflex. Urine analysis revealed protein (±) and urine pH 8.0. Serum electrolyte showed K 1. 8 mEq/L and Na 144 mEq/L. Blood pH,Iwas 7.44. Plasma renin activity was 140ng/%.
Retroperitoneal pneumogram showed enlarged left adrenal gland. And so under the diagnosis of primary aldosteronism, she was explored and found to have an adenoma in the left adrenal gland. After removal of the left adrenal gland, symptoms and signs were markedly improved.
